- QUALIFICATIONS:
- GED or High School education required.
- Two years experience in working with frail elderly and/or disabled preferred.
- Must be CPR certified within three months of employment.
- Provide proof of absence of tuberculosis in a transmissible form.
- Valid New Mexico driver's license and "clean" driving record.
- RESPONSIBILITIES:
- Acquire and apply ongoing training and education in aging and disabled populations' needs.
- Assess individual client strengths, needs, and preferences and apply these to client interventions.
- Maintain a client centered focus.
- Maintain a professional attitude, and work effectively under stress.
- Maintain a solution-based focus, and a flexible, cooperative and team-oriented, attitude.
- Understand the purpose of therapeutic programming; understand diagnoses, goal formulation, and implementation of goals on an individual and program basis.

- Communicating on a 1:1 basis with all populations of clients served.

- Communicating with family members, caregivers and other service providers.
- Communicating with the public (including those persons who share space at the facility).
- Maintaining confidentiality.
- Participating in staff meetings, in-service training, and other relevant trainings.
- Practicing environmental/personal safety and sanitation.
- Administering CPR, activating agency’s or emergency response plan.
- Assist clients with self-medication in accordance with the New Mexico Board of Pharmacy.
- Transport participants as needed.

- TRANSPORTATION:
- Transport participants to and from the day care program and on field trips as needed.
- Provide for the safe transportation of participants and assist them with boarding on and off the van in a safe manner.
- Intervene with participants exhibiting inappropriate or problematic behavior and/or signs of illness while on the van.
- Perform first aid assistance during an emergency and activate emergency medical system as needed.
- Report any incident/accident/problem occurring on the van while in the process of transporting participants.
- Perform preventative maintenance checks on van and report all problems of operation to immediate supervisor.

- Ability to communicate with different population groups served in the program.
- Willingness and ability to acquire and apply ongoing training and education regarding aging and disabled populations.
- Ability to respond to participants' needs and act appropriately in emergency and/or crisis situations

- DUTIES:
- Assist in daily participant care and supervision
- Assist in planning and implementation of daily activities.
- Take responsibility for maintaining a safe, caring, and supervised environment for the participants.

- Work as a team member and fill in with other duties, as needed, and other duties as assigned.
- Prepare and serve between meal supplements.

- Associates Degree preferred.
- One year experience working with population of program for (PSI, SDMI; for ISB, DD; etc.).
- Must be CPR and First Aid certified within three months of employment.

- Willingness and ability to acquire and apply ongoing training and education in aging, mentally ill, and disabled populations.
- RESPONSIBILITIES:
- Preparing and serving between meal supplements as needed.
- Maintaining a safe, caring, and supervised environment for the Clients.
- Communicating with the Program Coordinator any supervisory needs of Program staff.
- Assisting Program Coordinator in evaluating employee performance.
- Providing leadership, motivation, and direction to staff in conjunction with the Program Coordinator to ensure efficient/effective operation of Program.
- Scheduling daily assigned duties.
- Observing and detecting initial signs of illness and/or problem behaviors.
- Developing and leading meaningful activities in program milieu including socialization, skill
- building, health and fitness, self-esteem, ADL skill development, and other such events.
- Work with Program Coordinator regarding activity calendar implementation.
- Participating in the development of individual service plans. Participation on review teams.
- Advocating for Clients in assigned group.
- ttending regular interdisciplinary treatment meetings for Program population & other clients in assigned group as directed by Program Coordinator.
- Daily assigned duties in the day care program.
- Coordinate the weekly client council meetings with the Program Coordinator.
- Coordinate and conduct monthly fire drills in the Program area.
- Assist Program Coordinator with van scheduling.
- Assume responsibility of managing program and supervision of program staff in the absence of the Program Coordinator.
- ABILITY TO:
- Understand the purpose of therapeutic programming; understand diagnoses, goal setting, and implementation of goals on an individual and program basis.
- Understand the importance of community integration of persons served.
- Work cooperatively with co-workers/teams within the agency, and other service providers.
- Communicate with different population groups served in the program.

- Establish and maintain relationships with persons who have developmental disabilities.
- ASSIST:
- Clients with Activities of Daily Living, including personal care, hygiene, toileting, after cleansing, etc.

- Monitoring therapeutic diets and assisting Clients at meals and snack times.
- Document Client progress, write incident reports, and interact with family members/caregivers on behalf of Clients.
- Be willing and able to transport Clients as needed.
- In maintaining areas of responsibility in accordance with all applicable regulatory and Agency requirements.
- TRANSPORTATION:
- Transportation of Clients to and from the day care program and on field trips as needed.
- Assisting with loading and unloading of Clients in a safe manner.
- Intervene with Clients exhibiting inappropriate or problematic behavior and/or signs of illness while on the van.
- Perform first aid assistance during an emergency and activate emergency medical system as needed.
- Report an incident/accident/problem occurring on the van while in the process of transporting Clients.
- Perform preventative maintenance checks on van and report all problems of operation to immediate supervisor.
- TRAFFIC CITATION:
- Report all traffic citations received on or off the job to immediate supervisor within two working days of receipt of violation.
- If license is suspended, report of such suspension must be made immediately to supervisor.
- Perform other duties as assigned by supervisor.
