Share Your Care is dedicated to meeting all of the Adult Day Service needs of its participants and their families. At enrollment, a comprehensive assessment of needs is completed and an individualized service plan is developed to meet these needs. Share Your Care has certain exclusionary criteria in place for its programs, meaning there may be some medical needs/treatments that cannot be met. Please call our professional staff who will determine if we can meet your specific needs or not.
